What are biologics for Crohn's and ulcerative colitis?
Biologics are advanced medicines that target the immune system more precisely than older drugs like steroids or immunosuppressants. They are used for moderate-to-severe IBD and help reduce inflammation, control symptoms, and prevent complications.
As a consultant gastroenterologist in Dubai with over 25 years of experience, I have seen the evolution of biologics first-hand. From the days when infliximab was our only choice, to now, when multiple advanced therapies are available in the UAE — patients here have access to the same options as leading international centers.
Which biologics are available in Dubai / UAE?
For Crohn's disease:
- Infliximab (Remicade; biosimilar: Remsima) - IV infusion
- Adalimumab (Humira; biosimilar: Hyrimoz) - subcutaneous injection
- Ustekinumab (Stelara) - IV induction → SC maintenance
- Vedolizumab (Entyvio) - IV infusion
- Risankizumab (Skyrizi) - SC injection
- Guselkumab (Tremfya) - SC injection
- Upadacitinib (Rinvoq) - oral JAK inhibitor
For Ulcerative colitis:
- Infliximab (Remicade; Remsima)
- Adalimumab (Humira / Hyrimoz)
- Golimumab (Simponi) - SC
- Vedolizumab (Entyvio) - IV
- Ustekinumab (Stelara) - IV → SC
- Mirikizumab (Omvoh) - IV → SC
- Upadacitinib (Rinvoq)
What's the difference between infusion and injection biologics?
Infusion (IV): given in hospital or infusion centers (e.g. infliximab, vedolizumab, mirikizumab).
Injection (SC): taken at home or in clinic (e.g. Humira/Hyrimoz, Skyrizi, Tremfya, Simponi).
Some biologics start with IV induction for rapid effect, then switch to SC injections for long-term convenience.
What checks are needed before starting biologics?
- Blood tests: CBC, kidney, liver, inflammatory markers
- Infection screening: TB, hepatitis B & C, HIV
- Vaccinations: flu, pneumococcal, shingles, hepatitis A & B, HPV, COVID boosters
- Pregnancy testing if applicable
First doses are given under supervision in JCI-accredited centers that follow strict workflows — from proper medicine storage through to infusion safety and emergency readiness.
What are the common side effects of biologics?
Biologic medications are generally safe, but side effects can vary depending on the medicine and how it’s given.
Infusions (IV):
Headache, tiredness, flushing, mild rash.
Rarely, allergic reactions (e.g. swelling, breathing difficulty).
Injections (under the skin):
Redness, swelling, or itching at the injection site.
Sometimes, mild flu-like symptoms.
General risks:
Slightly higher chance of infections (e.g. colds, shingles).
Rarely, changes in liver tests, skin rashes, or nerve-related symptoms.
Important: This is not a full list of side effects. Your treating doctor will explain what to expect based on your condition and treatment.
What are biosimilars — and are they safe?
Biosimilars are highly similar versions of original biologics. They undergo rigorous testing for safety and effectiveness. Examples in the UAE include Remsima (for Remicade) and Hyrimoz (for Humira). They are approved by regulators and provide safe, effective treatment options with wider access.
