By Dr Pranab Gyawali, Consultant Gastroenterologist

This week, I saw a patient who’s been looking into Curcumin and Qing Dai as part of their ulcerative colitis treatment plan. It reminded me how often this topic now comes up in clinic — and that I’ve been meaning to explain what these natural compounds actually are, what the research says, and how they might fit into treatment for inflammatory bowel disease.

Q & A

What are Curcumin and Qing Dai?

Curcumin is the active anti-inflammatory compound found in turmeric. Qing Dai, also known as Indigo naturalis, is a herbal extract. A standardized combination of both (CurQD) has been tested as an add-on to usual treatment in active ulcerative colitis.

What did the 2024 CurQD study show?

In an 8-week randomized, placebo-controlled trial of 42 adults with active ulcerative colitis, CurQD added to standard therapy achieved higher rates of improvement than placebo:

  • Clinical remission: 50% vs 8%
  • Endoscopic improvement: 75% vs 20%

All participants continued their usual UC medications; this was an add-on, not a replacement.

How might they work?

Curcumin and Qing Dai are thought to act via the AhR pathway in the gut lining, helping calm excessive immune activity and supporting healing.

Is it safe and available?

In the study period, side effects were similar to placebo. However, the studied CurQD was pharmaceutical-grade. High or unregulated doses — especially of Qing Dai — may carry risks. Availability is limited and cost can be higher than typical supplements. Please discuss any plan to start supplements with your gastroenterologist.

What about Crohn’s disease?

Evidence is currently weaker. A postoperative curcumin trial in Crohn’s did not show a clear benefit. Most supportive data pertain to ulcerative colitis.
