Iron deficiency anemia (IDA) is one of the most common reasons patients visit me in Dubai. While low iron is often linked to diet or heavy periods, in many cases there’s an underlying gut cause — hidden bleeding, inflammation, or malabsorption. As a gastroenterologist, my role is to investigate and treat not just the anemia itself, but the reason behind it.

What Is Iron Deficiency Anemia?

Iron deficiency anemia occurs when your body doesn’t have enough iron to produce healthy red blood cells. Symptoms may include:

  • Tiredness and fatigue
  • Pale skin, brittle nails, hair loss
  • Shortness of breath or palpitations
  • Dizziness or brain fog

Common Causes of Iron Deficiency in Dubai

  • Heavy menstrual bleeding
  • Pregnancy and increased nutritional needs
  • Dietary factors (low iron intake, vegetarian/vegan diet)
  • Long-term PPI use (acid-suppressing tablets) reducing absorption
  • Hidden gastrointestinal blood loss (ulcers, polyps, hemorrhoids, inflammatory bowel disease)
  • Small bowel bleeding (sometimes only detected by capsule endoscopy)

When Does Iron Deficiency Need a Gastroenterology Work-Up?

In certain patients, iron deficiency requires a deeper investigation:

  • Men of any age
  • Women after menopause
  • Patients with recurrent or unexplained anemia
  • IDA with positive stool blood tests (FIT/FOBT)
  • IDA with gut symptoms (pain, diarrhea, blood in stool)

Tests We Use

Treatment Options

  • Oral iron tablets (often first line)
  • Intravenous (IV) iron if absorption is poor or side effects are strong
  • Dietary support – optimizing iron intake and absorption
  • Treating the cause – ulcers, bleeding, inflammation, or celiac disease

When Capsule Endoscopy Helps

Capsule endoscopy is often the next step when standard scopes are normal but anemia persists. It can identify:

  • Small bowel ulcers
  • Vascular lesions (angioectasias)
  • Crohn’s disease affecting the small intestine
  • Hidden bleeding sources

Do I always need a scope for iron deficiency?
Not always — but in men, post-menopausal women, or those with unexplained recurrent anemia, scopes are strongly advised.

What ferritin level is considered low?
Ferritin under ~30 μg/L is usually abnormal; higher thresholds are used in inflammation.

Can I get IV iron in Dubai?
Yes — I arrange IV iron when necessary, while investigating the cause.

Can I just take iron tablets?
Tablets may help temporarily, but without finding the cause, anemia often returns.
