If you have been advised to start a biologic for Crohn’s disease, one of the most important questions is not just which drug, but when to start it.
A recent 2026 study on vedolizumab (Entyvio) highlights something clinically important. The same treatment may work very differently depending on when it is introduced.
What matters here is not just the drug, but the timing of when it is used.
What is vedolizumab and how does it work?
Vedolizumab is a gut-selective biologic therapy used in Crohn’s disease and ulcerative colitis. It blocks α4β7 integrin, reducing inflammatory cell movement into the gut lining.
Vedolizumab works mainly in the gut, which is why timing and patient selection matter in Crohn’s disease.
This gut-selective action is why it is often preferred in patients where minimizing systemic immune effects is important.
What did the 2026 study show?
A 2026 study published in The Lancet Gastroenterology and Hepatology showed that patients treated earlier in their disease course had significantly higher rates of deep remission compared to those treated later.

Why does timing matter?
Crohn’s disease evolves over time. Early inflammation is often more responsive, while later disease may involve structural damage and fibrosis, making treatment less effective.
The same biologic can therefore produce very different outcomes depending on when it is used.
Does this apply to all Crohn’s patients?
No. Some patients have genuinely mild Crohn’s disease and can be safely monitored.
Others may appear mild but have features suggesting the disease is likely to progress. In these patients, earlier biologic therapy may improve outcomes.
