Patients with ulcerative colitis frequently ask about diet, probiotics, and gut health. While many probiotics are marketed for digestive symptoms, very few have been studied in inflammatory bowel disease.
One strain continues to stand out — not because it is new, but because it has been studied for more than 100 years: E. coli Nissle 1917.
What is E. coli Nissle 1917?
E. coli Nissle 1917 is a non-pathogenic strain of Escherichia coli identified during World War I, isolated from a soldier who remained free of dysentery when many others were affected.
Why is this probiotic still discussed in ulcerative colitis?
Clinical studies show that in patients with mild ulcerative colitis in remission, E. coli Nissle 1917 can help maintain remission and in some trials performed comparably to mesalazine (5-ASA).
It is the only probiotic referenced in European Crohn’s and Colitis Organisation (ECCO) guidance as an alternative option for remission maintenance in selected patients.
How does E. coli Nissle 1917 work?
- Competes with potentially harmful gut bacteria
- Supports the intestinal barrier
- Modulates immune activity at the colonic lining
Is this probiotic suitable for all UC patients?
No. It is not a treatment for active flares and is not suitable for all patients. Disease severity, immune status, nutritional deficiencies, and concurrent therapies must be considered.
Why are we still relying on a 100-year-old probiotic?
This is both fascinating and frustrating. Despite major advances in microbiome science, we still have very few bacteria with reliable clinical evidence in ulcerative colitis.
With modern tools such as AI-based microbiome modelling, it feels likely that the next major breakthrough in UC care will come from a deeper understanding of the gut microbiome.
