This video came from a recent conversation with a colleague from London who’s currently visiting Dubai. He shared a case of a young patient with Crohn’s disease who responded remarkably well to an elemental diet — a strict, liquid-only nutrition plan used in certain medical situations.

This is not to promote the diet, as it’s extremely hard to maintain. But there are times when your doctor might suggest it — particularly during a flare or while other treatments are being planned. It reminded me of something important — the role of diet in inflammatory bowel disease (IBD).

What Exactly Is the Elemental Diet?

The elemental diet provides nutrients in their simplest forms: proteins as amino acids, carbohydrates as simple sugars, and minimal/medium-chain fats. Because it’s pre-digested, absorption is quick and the bowel can rest and recover while nutrition remains complete.

How Effective Is It for Crohn’s Disease?

In children, remission rates are typically 60–85% and often comparable to corticosteroids. In adults, 30–60% achieve remission, with adherence being the main challenge. Even so, clinical markers and mucosal healing improve when used under specialist supervision.

Guideline Perspective – AGA Clinical Practice Update 2024

The American Gastroenterological Association (AGA) 2024 Clinical Practice Update on Diet and Nutritional Therapies in IBD recognises exclusive enteral nutrition as an effective therapy for inducing remission in Crohn’s disease. Evidence is strongest in children; in adults it may be considered as a steroid-sparing or bridge therapy in selected cases.

  • Reduces irritation: fewer complex food antigens reach inflamed mucosa.
  • Shifts the microbiome: toward a less inflammatory balance.
  • Provides gentle nutrition: maintains intake when appetite is low.
