What is a Urea Breath Test?

A urea breath test is a non-invasive diagnostic test used to detect infection with Helicobacter pylori, a bacterium found in the stomach that can cause gastritis, peptic ulcers, and increase the risk of stomach cancer. The test works by identifying urease, an enzyme produced by H. pylori that breaks down urea into carbon dioxide. By measuring labelled carbon dioxide in the breath, clinicians can confirm whether the infection is present.

When is a Urea Breath Test Recommended?

Healthcare providers may recommend the test in the following situations:

  • Unexplained upper abdominal pain or indigestion
  • Suspected or confirmed peptic ulcer disease
  • Persistent nausea, bloating, or early satiety
  • To confirm eradication of H. pylori after treatment
  • Evaluation of chronic gastritis or dyspepsia in younger patients

How is a Urea Breath Test Performed?

The test is performed after a short period of fasting and temporary discontinuation of certain medications. The patient first provides a baseline breath sample. They then drink a urea solution containing a harmless labelled compound. After 20 to 30 minutes, another breath sample is collected. The samples are analysed to detect increased labelled carbon dioxide, indicating the presence of H. pylori.

What to Expect after the Test

The test is simple, quick, and allows immediate return to normal activities. Most people experience no side effects. Very rarely, mild nausea or bloating may occur from the test drink, but these symptoms resolve rapidly without treatment.

What are the Risks of a Urea Breath Test?

The test is extremely safe, but minor risks include:

  • Temporary gastrointestinal discomfort
  • Rare nausea after the solution
  • Unsuitable for very young children or pregnant women in some settings

What are the Benefits of a Urea Breath Test?

Clinical benefits include:

  • Accurate, non-invasive detection of active H. pylori infection
  • No need for endoscopy in most cases
  • Reliable confirmation of treatment success
  • Guides targeted therapy and reduces complications
